Enrol with us

Thrive Medical welcomes your enrolment.

**please ensure you fill out your Next Of Kin details if you complete the online enrolment as we need this to complete your enrolment**  Those who fill out the online enrolment will get an email within a week with your myIndici login; this confirms your enrolment.  We are working as quickly as we can to process the enrolments; please allow a week for this.

To ensure you are eligible for publicly funded health services in New Zealand, it is our responsibility to assess your identity and eligibility. When returning your completed Enrolment Form, please also provide us with the following documentation: (you can take a photo)

 

Proof of identity - Driver’s License or Passport (or Birth Certificate for under 16-year-olds). If no photo ID is available, then two supplementary forms of ID e.g. credit card, invoice with name, other forms of non-photo ID with name included

Proof of eligibility - New Zealand Passport or Birth Certificate. If not born in New Zealand, a copy of your Passport and relevant Visa.

Drivers license alone is not sufficient for proof of eligibility; and overseas passports need to have a valid 2-year Visa attached.

 

There are three ways you can complete your enrolment:

 

  1. Fill out the online form and upload your ID

    Or

  2. Print out this enrolment form and bring into the practice with your relevant ID

    Or

  3. Pop in to Thrive, 36 Domain Road, open Monday to Friday 8:30am to 4pm (closed for lunch 11:30am-12:30pm)

     

If you would like to enrol your whanau, please complete a separate Enrolment form for each person. If you have dependents under 16 years old, please sign the Authority Details section at the bottom of page two, on their behalf.

By enrolling with Thrive you will be included in the enrolled population of this practice’s Primary Health Organisation (PHO) Western Bay PHO, and your name, address and other identification details will be included on the Practice, PHO and National Enrolment Service Registers.

​It will take up to 2 weeks to receive your medical records from your previous GP. 

Please do not book a non-urgent appointment with us until your notes have arrived (allow 2 weeks)

We will have a daily walk-in clinic 8:30am-10am (no appointment needed) for urgent medical care

You will get an email (within a week) with instructions on how to login to the patient portal called myIndici
